How do I request care?

What happens after I submit a request?

The office reviews the request and follows up to discuss care needs, scheduling, service availability, and next steps.

Can a family member or guardian submit for someone else?

Yes. A family member, guardian, referral partner, or responsible contact may start the request process.

What services can I ask about?

You can ask about personal care, respite support, companionship, daily living assistance, supervision, and help understanding available options.

Can I request an appointment?

Yes. Use the appointment request page to suggest a preferred date, time, and reason for the call or visit.

Is my information private?

Submitted information is used to respond to care requests and coordinate next steps. Do not submit emergency information through the website.

What if this is urgent?

Call the office directly or contact emergency services if the situation is urgent, unsafe, or life-threatening.
